• First you need to send your email and state information in the box below
   
• Then to protect your privacy, you need to respond to a confirmation email you will receive from majordomo@healthpromotionadvocates.org.  Please follow the directions in the confirmation email to complete the sign-up process.

• Select your state Using the pull down list
• Choose "unsubscribe"
• Click on the "Submit" button

A message will appear in red letting you know that your message to unsubscribe was sent. Within one day you will receive a notice in your email that you have been removed from the list.  Please check the message you received to ensure that you were successfully unsubscribed.  The unsubscribe will not be complete if you selected a different state than when you subscribed.  If you subscribed to multiple states you will need to perform an unsubscribe separately for each state for which you signed up.
